5 Whys: Getting to the Root of a Problem Quickly

The 5 Whys is a tool that can be used in troubleshooting, quality improvement and problem solving, but it is best for simple or moderately difficult problems. When a problem arises, simply keep asking the question “why” until you reach the underlying source of the problem, and until a robust counter-measure becomes apparent.
