MAPS – Volunteer Home Visit Data Capture Tool [Nigeria]
This is a tool from the Nigeria Malaria Action Program for States (MAPS). Community Volunteers fill this out during home visits in order to collect data. It is used by over 1,000 volunteers to report daily house to house visits in 7 states in Nigeria.
The form can be used to analyze behavior change at the household level by comparing the first visit and follow up visit to determine the changes that happened in the household as a result of the first visit. There is a home visit summary form, as well as a home visit data form, and explanatory guidelines for each.
